Senior Outreach Nurses Services Provided
The Nevada County Senior Outreach Nurses provide health services to county residents 60 years and older. The nurses provide:

• Comprehensive health and safety assessments
• Continued in home follow up
• Fall prevention education
• Health screenings
• Medication education/management resources

Secondly, they provide outreach such as community based blood pressure clinics, flu shots in conjunction with Public Health, and they participate in local health/wellness programs.

As a senior outreach nurse we visit people over 60 who have been referred to us by various sources including themselves, family members, adult protective services and other agencies. We make a visit to their home and assess their health needs and level of safety in the home. We help them with some of their needs with referrals to other services and see if they are able to take their medications correctly etc. Usually if someone goes on hospice we no longer keep them on our caseload as they receive full services through hospice. We have 3 nurses who do this under the Adult Services Department of Nevada County. Our number is  530-265-1470.
